When should a team doctor run out for on-field assessment for concussion as per ICC?

Several former players questioned Ravindra Jadeja not being assessed immediately after being hit on the head in the first T20I against Australia.
Concussion is defined as a ‘traumatic brain injury induced by biomechanical forces. It typically results in a rapid onset of neurological impairment that resolves spontaneously, although in some cases, clinical features may evolve over time.
As per ICC, a team doctor/physio should run out for an on-field assessment if called by the umpire or players. They can also assess the player at the end of the over in case the player resumes play.
In such circumstances, the player should be immediately removed from the match with the diagnosis of concussion already made.
